Comarch HomeHealth 2.0 is an application for a tablet or phone with integrated measuring devices that allows remote care of the patient outside the medical facility. It is intended for individual patients suffering from chronic illness or after hospital treatment, and for nursing homes or community nurses. The set of appropriate equipment can be modified according to the patient’s needs, such as for pulmonary patients or patients diagnosed with diabetes.

Treatment Schedule

The patient receives a schedule in the app that is individually tailored to their needs and medical condition, as prescribed by the doctor. This schedule includes tasks such as measurements, medications, surveys, and other activities that the patient should complete as part of their care pathway. This allows the patient easy access to their treatment plan from anywhere and at any time, making it easier to follow the doctor's recommendations. Such a personalized plan enables the patient to regularly and systematically complete the assigned tasks, which increases their engagement in the treatment process and leads to better therapy outcomes.

Reminder Function

The app sends daily reminders to the patient to complete a specific task within a designated time window.   • Ordering a telemonitoring service

    The doctor orders remote monitoring of the patient outside the medical facility and adjusts its course. Sets a schedule of tasks, drug doses, personalizes the ranges of measurement standards, orders medical surveys and selects appropriate educational materials.

  • Patient at home

    The patient performs tasks ordered by the doctor, which the application reminds him of every day. The results from the measuring devices are automatically sent to the application. All data collected from the patient are sent in real time to the Comarch e-Care 2.0 telemedicine platform, where they are automatically analyzed and monitored by the medical care team.

  • Report with recommendations

    After the monitoring period, the doctor can generate a report from the service along with recommendations and make it available to the patient. The patient also has access to all of his historical results in the Comarch HomeHealth 2.0 application.
