Heart failure is one of the greatest challenges in modern medicine, affecting millions of patients worldwide. Effective treatment relies on regular condition monitoring, rapid response to changes, and seamless communication between patients and medical teams. But how can these goals be achieved in everyday practice?
The ‘Polish Mother’s Memorial Health Centre’ Institute in Lodz, in partnership with the University of Stavanger and primary care units (POZs), launched a pilot telemedicine model for cardiac patients based on Comarch solutions. The project utilized the Comarch e-Care 2.0 telemedicine platform and the Comarch HomeHealth 2.0 mobile application, enabling ongoing monitoring of key health parameters—such as blood pressure, heart rate, and weight—directly from patients’ homes.
Thanks to these state-of-the-art telemedicine solutions, patients not only benefited from remote care, but also received rapid responses from medical teams whenever alarming results appeared. Survey results showed that 74% of participants were satisfied with this form of health monitoring, and 70% expressed a willingness to continue using telemedicine in the future.
